Nystagmus atlas
Examination15 nystagmus types · animationBárány classification 2019Last reviewed: October 2026
Nystagmus is the most informative sign in the vestibular examination and the hardest to interpret. In this atlas you can watch each type move in both eyes, in nine gaze positions, with and without fixation, and see the shape of the slow phase on the eye-position trace.
At a glance
Nystagmus starts with a pathological slow drift (slow phase); the quick phase is a corrective saccade that brings the eye back. It is named after the direction of the quick phase.1,2
Jerk nystagmus has slow and quick phases; pendular nystagmus is a sinusoidal oscillation with equal velocity in both directions.1
The slow-phase shape reveals the mechanism: constant velocity (vestibular imbalance), decreasing velocity (impaired gaze holding), increasing velocity (infantile nystagmus).1,2
Peripheral vestibular nystagmus is direction-fixed, horizontal-torsional and suppressed by fixation; pure vertical, pure torsional, direction-changing or fixation-resistant nystagmus suggests a central cause.1,4
Movements in the atlas are schematic; amplitude and frequency were chosen for teaching.
Atlas
Choose a nystagmus type. In the nine-square gaze map, tap one and the eyes look that way; the arrows show the expected quick phase in each position. Some types show a test button (head shaking, Dix-Hallpike, covering an eye). The atlas uses the examiner’s view: screen left is the patient’s right.
Nystagmus in the primary position without a stimulus. Assess it with and without fixation.1
Gaze-evoked nystagmus
Nystagmus that appears only on eccentric gaze, beating in the direction of gaze; it indicates impaired gaze holding (neural integrator).1,2
Alexander’s law
Peripheral vestibular nystagmus increases with gaze toward the quick phase and decreases with gaze away; its direction does not change.2
Fixation suppression
Peripheral vestibular nystagmus is markedly reduced by visual fixation; Frenzel goggles or video-oculography remove fixation.1
Provoked nystagmus
Nystagmus brought out by head shaking, position, hyperventilation, Valsalva, sound or vibration.1
Physiological end-point nystagmus
Low-amplitude, symmetric nystagmus at extreme lateral gaze that fades within seconds; it is not pathological.1
How to look for nystagmus
Primary position, with fixation: look for spontaneous nystagmus while the patient fixes a distant target.
Without fixation: use Frenzel goggles, video-oculography or an ophthalmoscope. During fundoscopy covering the other eye removes fixation; the horizontal and vertical movement seen at the optic disc is opposite to the true eye movement, while torsion has the same direction.2
Gaze positions: wait at least a few seconds in right, left, up and down gaze; avoid extreme gaze, where physiological nystagmus may appear. Check whether the direction changes and whether Alexander’s law holds.1,2
Provocative tests: head shaking, positional tests (Dix-Hallpike, supine roll, head hanging) and, if needed, hyperventilation.1
Duration: observe spontaneous horizontal nystagmus for at least 2 minutes; periodic alternating nystagmus is missed in a brief examination.2
Description: record the direction (quick phase), plane (horizontal, vertical, torsional), waveform (jerk or pendular), equality between the eyes, and change with gaze and fixation.
