The semicircular canals answer "how far did I turn?"; the utricle and saccule answer "which way am I moving, and which way is down?" Together they describe the full motion of the head in space.
At a glance
The utricular macula lies close to horizontal and the saccular macula close to vertical; together they encode three-dimensional linear acceleration and tilt relative to gravity.1
Hair-cell orientation reverses across the striola, so a single macula can distinguish every direction.1,2
The otoliths alone cannot tell tilt from linear acceleration; the brain resolves the ambiguity by combining them with canal signals, a computation carried out in the cerebellum.3,8
cVEMP tests the saccule–inferior vestibular nerve pathway, oVEMP the utricle–superior vestibular nerve pathway; the subjective visual vertical shows static otolith balance.4,5
Anatomy
The macula, the sensory epithelium of each otolith organ, carries hair cells and supporting cells. Above the hair bundles lies a gelatinous layer topped with otoconia. Otoconia are calcium carbonate crystals in the calcite form, assembled on a protein scaffold; their most abundant protein is otoconin-90. Because the crystals are denser than endolymph, gravity and linear acceleration shift the otolithic membrane and bend the hair cells.6,1
Figure 1 · Macula and striolaHair cells face toward the striola in the utricle and away from it in the saccule. This mirror-image layout lets each macula distinguish tilt and linear acceleration in every direction.1,2
The utricle is supplied mainly by the superior vestibular nerve and the saccule mainly by the inferior vestibular nerve. This split makes it possible to tell with VEMPs which division is involved in vestibular neuritis.4,12
Type I hair cells and irregularly firing afferents in the striolar region are especially sensitive to rapidly changing stimuli; clinical stimuli such as sound and bone vibration activate these afferents selectively.2,7
Physiology
Tilt or acceleration?
What physics calls the equivalence principle applies to the otoliths: tilting the head to the right and accelerating to the left produce the same shear force on the macula. The otoliths alone cannot separate the two. Using rotation information from the semicircular canals, the brain continuously computes the direction of gravity in the head and interprets the remainder as true linear motion. Traces of this "internal model" have been shown in brainstem neurons and in Purkinje cells of the posterior cerebellar vermis (nodulus–uvula).3,8
Otolith reflexes
Linear VOR: When the head translates sideways or forward, the eyes turn the other way at a speed scaled to target distance.1
Ocular counter-roll: When the head tilts sideways the eyes tort the other way; in humans this offsets only a small part of the tilt.1
Posture: The lateral vestibulospinal tract carries mainly otolith input and adjusts antigravity muscles (see vestibular nuclei).1
Did you know?
The saccule is not a hearing organ, yet it responds to loud sound. cVEMP uses exactly this "vestigial" sound sensitivity: a loud tone delivered to the ear starts a signal in the saccule that is recorded as a brief inhibition of the ipsilateral sternocleidomastoid muscle.4,7
Air-conducted sound or bone vibration; inhibitory p13–n23 wave in the SCM
Absent or small after unilateral loss; low threshold in superior canal dehiscence4
oVEMP
Utricle → superior vestibular nerve → contralateral inferior oblique
Bone vibration or sound; excitatory n10 wave beneath the eye (on upgaze)
Lost in superior-division vestibular neuritis; enlarged in dehiscence4,7
Subjective visual vertical
Balance of static otolith and graviceptive pathways
Setting a line to vertical in darkness or in the bucket test
Tilts toward the lesion in acute unilateral peripheral or lower brainstem lesions5,13
At the bedside
The bucket test asks the patient to set a straight line drawn on the bottom of a bucket to where they believe vertical is, measuring the subjective visual vertical without special equipment. In the acute vestibular syndrome a clear tilt indicates a peripheral or central (lower brainstem) imbalance; its direction and the accompanying signs narrow down the lesion site.5,13

Key finding
BPPV
Otoconia dislodged from the utricle enter a semicircular canal
Positional nystagmus; otoconial metabolism and vitamin D deficiency have been linked to recurrence10,9
Vestibular neuritis
The utricle is involved with the superior division, the saccule with the inferior division
Loss of oVEMP or cVEMP; together with vHIT the affected division is mapped12,4
Superior canal dehiscence
The third window lets sound and vibration drive larger flows in the labyrinth
Changes in VEMP responses and an upward shift of the best-response frequency have been reported4
With age, otoconia fragment and are lost. This is one possible explanation for the rising frequency of BPPV and the decline in otolith function in older people.6,10
