Biliyor muydunuz?
Bucket testi

Subjective visual vertical and the ocular tilt reaction
Examination and testsRoll plane · labyrinth to cortexBucket test · topography toolLast reviewed: October 2026
Holding a bright line upright in a dark room seems simple, but it needs the signals from the two utricles to be balanced in the brainstem, thalamus and cortex. When that balance fails, the eyes, the head and perception all tilt the same way: the ocular tilt reaction. The subjective visual vertical (SVV) is the most sensitive measure of this imbalance and shows both the side and the level of the lesion.1,4
At a glance
SVV tilt is the most frequent and sensitive clinical sign of unilateral vestibular lesions from the labyrinth to the parieto-insular cortex; it is abnormal in 94% of acute unilateral brainstem lesions.2,4
The rule is simple: labyrinthine and pontomedullary lesions tilt it ipsiversively, pontomesencephalic lesions contraversively; thalamic and cortical lesions can tilt it either way, but less.1,4
The bucket test needs no special equipment and agrees closely with the hemispheric dome (r = 0.90); healthy people deviate by 0.9 ± 0.7° on average.3
In skew deviation the SVV is tilted with both eyes open; in fourth nerve palsy it is normal with both eyes open. The difference helps separate the two at the bedside.8,10
How do we find vertical?
The main organ that senses the direction of gravity is the utricle. Signals from the two utricles meet in the vestibular nuclei, cross at pontine level, ascend in the MLF to the interstitial nucleus of Cajal (rostral midbrain), and reach the vestibular cortex through the thalamus. When activity falls on one side of this graviceptive pathway, the brain behaves as if the head were tilted: it tries to “correct” the eyes and head, and perceives vertical in the wrong place.1,4
The SVV is measured by having the patient set a bright line to vertical in complete darkness (or inside a bucket that removes visual cues). In the dome study, deviation greater than 2.5° was taken as pathological; in 111 patients with acute unilateral brainstem lesions the mean tilt was 8.1 ± 5°.2
Did you know?
The brain has a “visual vertical” and a separate “postural vertical”. In pusher syndrome, patients feel upright when their body is tilted about 18° toward the side of the lesion, and push themselves that way with their healthy limbs; their visual vertical, by contrast, is usually normal. This dissociation suggested that humans have a second graviceptive system.11
Bucket test
The patient looks into a bucket and turns the line on its base to where it seems vertical; the examiner reads the deviation from a protractor on the outside. The bucket agrees closely with the dome (r = 0.90), is highly repeatable (r = 0.92), and finds a mean tilt of 8.3 ± 5.0° in patients (vestibular neuritis, brainstem infarction, neurectomy).3 In a series of 100 healthy people, 90% stayed within 2.6° and 95% within 3.4°.15
In the simulation below, choose a patient and run six trials. Each trial starts the bucket from a random angle, and the patient turns the line to where vertical seems to be.

Ocular tilt reaction
The ocular tilt reaction (OTR) is the combined response to a tone imbalance in the roll plane. Its four components point the same way:1,8
Component
Finding
How to look
Head tilt
The head tilts toward one shoulder
Observation; the patient usually does not notice
Skew deviation
The eye on the side of the head tilt is lower (hypotropia)
Alternate cover test; vertical refixation
Ocular torsion
The upper poles of the eyes rotate toward the tilt
Fundus photography or fundoscopy; macula–disc relationship
SVV tilt
Perceived vertical tilts the same way
Bucket or dome test
The OTR can be complete or partial. In peripheral lesions it is usually partial and brief;7 rostral midbrain lesions give a complete, marked, contraversive OTR.6 Skew deviation is one of the central signs in the HINTS examination.13
At the bedside
In the acute vestibular syndrome, the SVV or subjective visual horizontal is tilted toward the lesion in almost every patient with vestibular neuritis (97.6%), but in only a quarter of patients with stroke (24.3%). An absent tilt, or a tilt to the other side, therefore raises the likelihood of a central lesion.12
Lesion topography
Choose the lesion site and side. The patient is drawn from the examiner’s view: the patient’s right is on the left of the screen. Ocular torsion is exaggerated so that it can be seen.
In acute vertical diplopia, the two conditions most often confused are skew deviation and trochlear nerve palsy. The first indicates a central graviceptive imbalance, the second a lesion of an ocular motor nerve.
