Biliyor muydunuz?
Hastalar kendi göz hareketlerini, kalp atımlarını, çiğneme ve ayak seslerini duyduklarını anlatır. Kemik yolu öyle duyarlı hale gelir ki bazı hastalar ayak bileğindeki kemiğe konan diyapozonu kulaklarında duyabilir. Odyogramda bunun karşılığı, sıfırın altına inen kemik yolu eşikleridir.3,12

Tanı
Odyoloji
Tipik bulgu, alçak frekanslarda belirgin, 2000 Hz’in üstünde azalan hava–kemik aralığıdır.

The inner ear has two windows: sound enters at the oval window and leaves at the round window. When a third opening forms in the bony capsule of the labyrinth, the patient may hear their own eye movements, become dizzy with loud sound, and have an audiogram that looks like otosclerosis. Superior semicircular canal dehiscence, described in 1998, is the best-known member of this family.1
At a glance
A third window diverts part of the sound and pressure energy away from the cochlea and into the vestibular organ: air-conducted hearing falls, bone-conducted hearing rises, and the canal is driven by sound and pressure.4
The Bárány criteria require three components: at least one symptom consistent with a third window, at least one physiological test showing it, and matching high-resolution CT.2
CT alone is not enough: dehiscence is reported in 2.1–10.7% of CT series but only 0.5–0.6% of post-mortem series. Without symptoms and a physiological test, a CT finding does not make the diagnosis.2,5,11
When a conductive-looking loss comes with a normal tympanogram and preserved acoustic reflexes, consider a third window before otosclerosis surgery.4
Mechanism
In an intact labyrinth, when the stapes moves inward, fluid can move in only one direction: through the cochlea to the round window. If there is another mobile area in the bony capsule, the fluid has a second exit. Part of the energy bypasses the cochlea and escapes through the opening, so air-conducted sound has less effect on the cochlea. With bone-conducted vibration, by contrast, the pressure difference between the two windows grows and the cochlea is driven more easily.4
Figure 1 · A third window: a new path for energy (schematic)Normally, sound energy entering at the stapes passes through the cochlea and leaves at the round window (blue). When there is an opening in the bony cover of the superior canal, part of the energy escapes into the canal and out of the opening (orange, dashed): air-conducted hearing falls and the canal is driven by sound and pressure. Bone-conducted vibration, by contrast, drives the cochlea more easily; bone conduction thresholds fall below normal.4,5
The same opening makes the superior canal sensitive to sound and pressure. Sound, or pressure in the ear canal, moves the endolymph of the canal and produces nystagmus in the plane of the canal (Ewald’s first law). Depending on the direction of the stimulus the canal is excited or inhibited; for example, Valsalva against pinched nostrils and against a closed glottis produce eye movements in opposite directions.1,3
Did you know?
Patients describe hearing their own eye movements, pulse, chewing and footsteps. Bone conduction becomes so sensitive that some patients can hear, in their ear, a tuning fork placed on the ankle bone. Its counterpart on the audiogram is bone conduction thresholds that fall below zero.3,12
Symptoms and criteria
In 2021 the Bárány Society published the first international criteria for superior canal dehiscence syndrome. The diagnosis needs at least one criterion from each of groups A, B and C, and the picture must not be better explained by another disorder.2
Group
Criterion
Clinical counterpart
A · Symptoms (at least one)
Bone conduction hyperacusis; sound-induced vertigo or oscillopsia (Tullio); pressure-induced vertigo or oscillopsia; pulsatile tinnitus
Autophony; hearing eye movements, footsteps, bowel sounds; brief dizziness with coughing, sneezing, straining or ear canal pressure2
B · Physiological tests (at least one)
Nystagmus in the plane of the superior canal with sound or pressure; bone conduction thresholds below zero at low frequencies on audiometry; enhanced VEMPs (low cVEMP threshold or high oVEMP amplitude)
Nystagmus watched with video-oculography; audiogram and VEMPs2
C · Imaging
Dehiscence on high-resolution temporal bone CT with multiplanar reconstruction in the plane of the superior canal and orthogonal to it
In the Massachusetts Eye and Ear series (116 patients, 161 ears), the commonest complaints were hearing loss (52%), vertigo (46%), pulsatile tinnitus (30%), autophony (27%) and oscillopsia (14%). Audiometry supported the diagnosis in 77% of evaluable ears and VEMPs in 56%.5
Mark your patient’s symptom, test and imaging findings. The diagnosis needs at least one criterion from each of the three groups; a dehiscence seen on CT alone is not the syndrome.

Diagnosis
Audiology
The typical finding is an air–bone gap that is largest at low frequencies and shrinks above 2000 Hz. Part of the gap comes from air-conduction loss and part from bone conduction being better than normal (0 to −20 dB HL). The tympanogram is normal and acoustic reflexes are preserved, whereas in middle-ear disease the reflexes are lost. On electrocochleography the SP/AP ratio can be raised and may normalise after surgery.4,9
VEMP
A third window makes the otolith organs more sensitive to sound: the cVEMP threshold falls and the oVEMP amplitude rises. In a series of 29 surgically confirmed patients, a 500 Hz oVEMP amplitude cut-off of 17.1 µV gave 100% sensitivity and 98% specificity; a cVEMP threshold cut-off of 85 dB nHL gave 86% sensitivity and 90% specificity.7 An oVEMP response at 4 kHz, where normal ears usually give none, also strongly suggests dehiscence.8
Imaging
Temporal bone CT with 0.5–0.625 mm slices should be reconstructed in the plane of the canal (Pöschl) and orthogonal to it (Stenvers). With 1 mm slices and only axial and coronal images the positive predictive value is 50%; with 0.5 mm slices and reconstruction in the canal plane it rises to 93%. Thin but intact bone can look dehiscent because of partial volume averaging.5,6
At the bedside
Before stapes surgery in a patient with a low-frequency air–bone gap, look at three things: the tympanogram, the acoustic reflex and VEMPs. If the reflex is preserved and VEMPs are enhanced, the problem is in the inner ear, not the middle ear. Unnecessary middle-ear operations have been reported where this distinction was missed.4

Lesion
Key feature
Posterior semicircular canal dehiscence
0.3–4.5% on CT, 0.2% at post-mortem; often linked to a high-riding jugular bulb. The audiological pattern resembles superior canal dehiscence5
Enlarged vestibular aqueduct
Mixed or sensorineural loss, usually from childhood; may worsen after head trauma. On CT the aqueduct midpoint is ≥1.5 mm (Valvassori) or ≥1 mm (Cincinnati)5
Cochlear–carotid dehiscence
Air–bone gap that grows toward low frequencies; pulsatile tinnitus5
X-linked stapes gusher (POU3F4)
Mixed loss in boys; deficient cochlear modiolus and a wide internal auditory canal. Risk of perilymph gusher in stapes surgery5
Perilabyrinthine fistula
Mostly erosion by cholesteatoma; the lateral canal is most often involved5
Bone disorders
Demineralisation of the otic capsule in otospongiosis, osteogenesis imperfecta and Paget’s disease5
Treatment
If symptoms are mild, explanation and avoiding triggers are often enough. Surgery is considered for sound- or pressure-induced vertigo, autophony or oscillopsia that disrupts daily life. The dehiscence is closed through the middle cranial fossa or a transmastoid approach (plugging, resurfacing or both). Round window reinforcement is also used, but its evidence is more limited.3
In a meta-analysis of hearing after transmastoid plugging (11 studies, 159 ears), air conduction thresholds did not change; bone conduction worsened by 3.5 dB on average. The plugged canal loses its function, which matters when planning surgery on the second side in bilateral dehiscence.3,10
