Research horizon

The history page tells where vestibular science came from; this page shows where it is going. Ongoing and recently completed studies are placed on one board by stage, each with its result, next step and source.
At a glance
Two device routes are advancing in bilateral vestibulopathy: the vestibular implant (first human results in 2021) and external noisy galvanic stimulation (nGVS).1,3
Two of the last large drug trials in Ménière’s disease were negative; the placebo arm improved markedly too (a 55% fall in vertigo days).6,7
The steroid question in vestibular neuritis is being addressed by the 404-patient Nordic PREVENT trial.9
Inner-ear gene therapy has had its first clinical success in hearing; there is no vestibular-targeted human trial yet.4
Research board
Each row is a research area and each column a stage. The marker shows the result: an open circle is ongoing, a filled circle positive, a half circle promising but not conclusive, a cross negative. Select a study to see the details. The empty last column is a finding in itself: none of these routes has yet entered routine clinical use.

What the board teaches
Placebo is powerful: Because attacks in Ménière’s disease can wane spontaneously, the placebo arm improves substantially too. With OTO-104 the difference was only 3 points (58% vs 55%); in BEMED attacks fell over time in all three arms. Uncontrolled series overstate the effect.6,7Evidence atlas →
Pilots inspire, they do not decide: the nGVS (11 patients) and galcanezumab (40 patients) studies point the way; large multicentre trials are needed to change clinical decisions.3,8
A company report is not a peer-reviewed paper: the SPI-1005 results are known so far only from a company report; they will be reassessed when the primary endpoint and p values are published.5
In diagnosis the bottleneck is expertise: portable video-oculography and AI aim to bring eye-movement examination to centres without a specialist.11,12
The future of the criteria
Nineteen documents have been published in the Bárány Society’s International Classification of Vestibular Disorders since 2009; the latest is a 2025 position paper on the classification’s achievements, challenges and future directions. The society’s classification committee is working on further documents; titles and dates have not been announced. When a new document appears, the relevant disorder page and criteria checker will be updated.13History of the criteria →
Methods and updates
Selection: studies in humans, completed or under way, with a chance of changing clinical practice were included; animal and cell studies are mentioned only when they lead a clinical path.
Source type: each card makes the source type clear: peer-reviewed paper, trial registry, meeting abstract or company report. Non-peer-reviewed sources should be read with caution.
Updates: the board is reviewed in the monthly literature screen; studies whose stage or result changes are logged in the release notes. You can suggest a study through the contact address.
