Her iç kulakta, başın açısal hareketini üç boyutta algılayan üç halka. Baş döndüğünde kanal içindeki endolenf eylemsizlik nedeniyle geride kalır, kupulayı saptırır ve tüylü hücreler bu sapmayı sinir sinyaline çevirir.
Her tarafta üç kanal vardır: lateral (horizontal), anterior (süperior) ve posterior. İki taraftaki kanallar üç eş düzlemli çift oluşturur: iki lateral kanal, LARP (sol anterior–sağ posterior) ve RALP (sağ anterior–sol posterior).1,2
Bir çiftte bir kanal uyarılırken eşi inhibe olur; beyin sapı iki taraf arasındaki farkı okur.2
Uyarılan kanal, gözü kendi düzleminde döndürür (Ewald'ın birinci yasası). Nistagmusun düzlemi bu yüzden hangi kanalın etkilendiğini gösterir.2,5
vHIT altı kanalı ayrı ayrı, kalorik test yalnızca lateral kanalı değerlendirir; pozisyonel testler BPPV'de etkilenen kanalı bulur.9,10
Anatomi

Three rings in each inner ear that sense angular head motion in three dimensions. When the head turns, the endolymph lags behind by inertia, deflects the cupula, and the hair cells turn that deflection into a nerve signal.
Each side has three canals: lateral (horizontal), anterior (superior) and posterior. Across the two sides they form three coplanar pairs: the two lateral canals, LARP (left anterior–right posterior) and RALP (right anterior–left posterior).1,2
When one canal of a pair is excited its partner is inhibited; the brainstem reads the difference between the two sides.2
An excited canal rotates the eyes in its own plane (Ewald's first law), so the plane of the nystagmus points to the affected canal.2,5
vHIT tests all six canals separately, the caloric test only the lateral canal; positional tests find the affected canal in BPPV.9,10
Anatomy
The canals are part of the membranous labyrinth inside the bony labyrinth of the petrous temporal bone and are filled with endolymph. Each canal has a widened end, the ampulla. The non-ampullated ends of the anterior and posterior canals join in a common crus, so the three canals open into the utricle through five openings.2
The lateral canal ampulla lies at the front end, the anterior canal ampulla at the front-upper end and the posterior canal ampulla at the lower-back end. In the upright and supine positions the posterior canal is the most gravity-dependent part of the inner ear, which is why otoconia dislodged from the utricle fall most often into the posterior canal and posterior canal BPPV is the most common type.10
Canal planes and pairs
Three-dimensional CT measurements show that the angles between canals on the same side are close to 90°: anterior–posterior 94.0 ± 4.0°, anterior–lateral 90.6 ± 6.2°, lateral–posterior 90.4 ± 4.9°. The lateral canal plane is tilted about 20° nose-up relative to Reid's plane (the plane through the infraorbital rim and external auditory canal). Paired canals of the two sides are not exactly coplanar, but the angle between them is small enough to treat them as coplanar in practice.1
Figure 1 · Canal planes, viewed from aboveSchematic. In this view the lateral canals lie close to the horizontal plane (front tilted up by about 20°). To test the LARP plane the head is turned about 45° to the right, for RALP to the left; the plane then lines up with the sagittal plane and the head is moved nose down or nose up.

Ampulla and cupula
The floor of the ampulla carries a ridge across the canal axis, the crista ampullaris. The stereocilia and kinocilium of its hair cells are embedded in the cupula, a gelatinous partition that reaches the roof of the ampulla. The cupula seals the ampulla; when endolymph flows along the canal the cupula bulges like a membrane and bends the hair cells.2,4
The cupula has almost the same density as endolymph, so it is normally insensitive to gravity and responds only to angular acceleration. When otoconia stick to it (cupulolithiasis) the cupula becomes heavy and stays deflected according to head position, so positional nystagmus lasts long.4,10
Hair cells depolarize (are excited) when the stereocilia bend toward the kinocilium and hyperpolarize (are inhibited) when they bend away. All hair cells on one crista face the same way, so each canal has one excitatory and one inhibitory flow direction.2
Ewald's laws
Figure 2 · Excitatory flow directionSchematic; ampulla on the left, the rest of the canal on the right. Kinocilia face the utricle in the lateral canal and the canal in the vertical canals.
First law: Nystagmus occurs in the plane of the excited canal and in relation to the direction of endolymph flow. Identifying the affected canal from the direction of nystagmus in BPPV rests on this principle.5,2
Second law: Ampullopetal flow in the lateral canal and ampullofugal flow in the vertical canals produce the stronger response, because afferent firing can rise over a wide range when excited but cannot fall below zero when inhibited.5,3
This is why, in lateral canal BPPV, the side with the stronger nystagmus on the roll test identifies the affected ear: in the geotropic type the stronger response comes when lying on the affected side, in the apogeotropic type when lying on the healthy side.10
Biomechanics
The canal–cupula system behaves like an overdamped torsion pendulum. Because of endolymph viscosity and the narrow canal lumen, at the mid frequencies that cover most everyday head movements cupular deflection is proportional to angular head velocity rather than acceleration. The canals therefore send the brain a head-velocity signal.4,3
During constant-velocity rotation the cupula returns to its resting position within a few seconds by its own elasticity; in humans this time constant has been estimated at about 4 seconds.6 Rotation sensation and nystagmus last longer because of velocity storage in the brainstem (see vestibulo-ocular reflex). When rotation stops abruptly the cupula deflects the other way, producing post-rotatory nystagmus.2

Key diagnostic feature
BPPV, canalolithiasis
Free otoconia move within the canal with head position; endolymph flow deflects the cupula
Positional nystagmus with latency and short duration, in the plane of the affected canal10
BPPV, cupulolithiasis
Otoconia stuck to the cupula make it heavy
Nystagmus with no or short latency that persists while the position is held10
Vestibular neuritis
Superior division: lateral and anterior canals; inferior division: posterior canal
Low vHIT gain and corrective saccades in the affected canals7,8
Superior canal dehiscence
The bone over the canal is thin or absent; a "third window" lets sound and pressure drive flow in the canal
Sound- or pressure-induced vertigo and nystagmus in the superior canal plane, autophony, low VEMP thresholds, thin-section CT11,12
Canal-specific testing: vHIT measures each of the six canals; the caloric test reaches only the lateral canal and is a very low-frequency stimulus; the Dix-Hallpike test brings the posterior (and less so the anterior) canal, and the supine roll test the lateral canal, into the plane of gravity.9,10
