HINTS+).7

The cerebellum is the calibrator of the vestibular system: it tunes and learns reflexes, holds gaze steady and computes the physics of motion. Disease of the vestibular cerebellum often shows first in eye movements, which makes the eye examination its most sensitive window.
At a glance
The flocculus and paraflocculus tune VOR gain through learning and support smooth pursuit and gaze holding; damage causes downbeat and gaze-evoked nystagmus and impaired VOR suppression.1,3
The nodulus and ventral uvula tune velocity storage and separate tilt from linear acceleration; damage can cause periodic alternating nystagmus and central positional nystagmus.1,5,6
Purkinje cells project directly and inhibitorily onto the vestibular nuclei; this is how the cerebellum tunes vestibular reflexes.4
Treatment evidence is growing: 4-aminopyridine is effective in downbeat nystagmus and episodic ataxia type 2; the genetic cause of CANVAS has been identified as RFC1.9,10,11

Lesion signs
Flocculus, paraflocculus
VOR gain adaptation, smooth pursuit, holding eccentric gaze, suppressing the VOR while tracking a target
Downbeat nystagmus, gaze-evoked and rebound nystagmus, saccadic pursuit, impaired VOR suppression1,2
Nodulus, ventral uvula
Tuning the duration of velocity storage, processing otolith signals, separating tilt from translation
Periodic alternating nystagmus, central positional nystagmus, perverted head-shaking nystagmus1,5,6
VOR gain is recalibrated when the image slips on the retina during head motion. The flocculus–paraflocculus and vestibular nuclei work together in this learning; the error signal reaches Purkinje cells through climbing fibres.3
Did you know?
VOR adaptation is one of the best-studied models of motor learning. With magnifying or minifying spectacles, VOR gain adapts to the new visual conditions within days; the same circuits bring the gain back when the spectacles come off.3
The cerebellum computes physics as well as reflexes: Purkinje cells in the nodulus–uvula combine canal and otolith signals into an internal model that separates head tilt from true linear motion (see otolith organs).5
Clinical syndromes
Downbeat nystagmus
One of the most common types of central nystagmus, linked to flocculus dysfunction. A series of 117 patients described many causes, led by cerebellar degeneration and idiopathic cases.8 It increases on lateral and downgaze and can cause oscillopsia and gait unsteadiness.2
At the bedside
The potassium-channel blocker 4-aminopyridine increases Purkinje-cell excitability. In a randomized double-blind crossover trial (27 patients), 5 mg four times daily lowered slow-phase velocity from 2.42°/s to 1.38°/s; 57% of patients responded, with idiopathic cases benefiting most.9
CANVAS and RFC1
The combination of cerebellar ataxia, neuropathy and bilateral vestibular areflexia. The most characteristic bedside sign is an impaired visually enhanced VOR: because both the VOR and pursuit are affected, the eyes move in steps when the head is turned slowly while the patient views a target.12 In 2019 a biallelic expansion of an intronic repeat in RFC1 was identified as a common cause of CANVAS and late-onset ataxia.11
Episodic ataxia type 2
Associated with CACNA1A mutations, it causes attacks of ataxia and vertigo lasting hours; downbeat or gaze-evoked nystagmus may be present between attacks. In a randomized trial 4-aminopyridine reduced attack frequency compared with placebo.10
Cerebellar stroke
Nodulus or flocculus infarction can present as an acute vestibular syndrome and mimic peripheral vestibulopathy; the head impulse test is normal and HINTS shows the central pattern (see HINTS+).7
