A two-division cable carrying signals from five receptors to the brainstem. Thanks to vHIT and VEMPs, the question "which division is involved?" can now be answered at the bedside and in the lab, changing diagnosis, prognosis and research questions.
At a glance
The superior division carries the anterior and lateral canals and the utricle; the inferior division carries the posterior canal and most of the saccule.1,6
The cell bodies lie in Scarpa's ganglion in the internal auditory canal; afferents form two functional classes, regular and irregular.2
The labyrinth is supplied by an end artery, usually from the AICA; ischaemia can affect hearing and balance together and may hide a central cause.4
Vestibular neuritis, vestibular paroxysmia and vestibular schwannoma are the main disorders at this level; in severe bilateral loss, vestibular implants that stimulate nerve branches are in clinical research.5,12,13,14
Anatomy
Together with the cochlear nerve, the vestibular nerve forms the eighth cranial nerve and splits into two divisions in the internal auditory canal. Knowing which receptors each division carries is the key to turning vestibular test results into an anatomical map.1
Figure 1 · Which receptor, which division?A small part of the saccule is also supplied by the superior division (dashed line). Superior-division involvement affects the lateral and anterior canals and the utricle; inferior-division involvement affects the posterior canal and saccule, which is why vHIT and VEMPs map the affected division.1,3,6
At the bedside
The most common pattern in vestibular neuritis is superior-division involvement: low vHIT gain in the lateral and anterior canals and loss of the oVEMP, with a preserved posterior canal and cVEMP. Isolated inferior-division involvement is rare and, because the caloric test stays normal, is missed unless vHIT and cVEMP are done.6,7
Afferents
Vestibular afferents fire continuously even at rest and fall between two extremes of firing regularity:2
Regular afferents: mostly from the peripheral zones of the receptors; they encode head motion linearly and steadily.
Irregular afferents: mostly from the centre of the crista and the striola, from calyx endings on type I hair cells; they are more sensitive to rapid change. Sound, bone vibration and galvanic stimuli activate them selectively, and they are the main source of VEMP responses.3
Did you know?
Even with the head perfectly still, the vestibular nerve is not silent: in the squirrel monkey, canal afferents fire at rest at an average of about 90 spikes per second. The brain reads motion from rises and falls in this baseline; after a sudden unilateral loss, the baseline activity on the healthy side creates a sense of rotation and nystagmus even with the head still.15,1
Blood supply
The labyrinthine (internal auditory) artery usually arises from the AICA and is an end artery without collateral supply. The anterior vestibular artery supplies the lateral and anterior canals and the utricle, the posterior vestibular artery the posterior canal and saccule, and cochlear branches the cochlea. This distribution explains why a vascular event can mimic vestibular neuritis and why hearing and vestibular loss occur together in AICA infarction.4
This is why the "plus" step of the HINTS examination, asking about new hearing loss, matters (see HINTS+).
The Bárány Society criteria require acute-onset sustained vertigo lasting at least 24 hours, spontaneous nystagmus of peripheral type (direction-fixed, suppressed by fixation), reduced VOR function on the side opposite the fast phase, and no central signs.5 The cause is uncertain: detection of HSV-1 DNA in the vestibular ganglion has supported the viral reactivation hypothesis.8
Methylprednisolone was shown to improve recovery of the caloric response at 12 months, with no added benefit from valacyclovir.9 Systematic reviews, however, found insufficient evidence for a long-term benefit of corticosteroids on symptoms and function.10 Early vestibular rehabilitation is the core treatment that speeds compensation.11
Vestibular paroxysmia
Characterized by brief, stereotyped, frequently recurring attacks of vertigo lasting seconds; neurovascular compression is thought to be responsible. The Bárány criteria include at least ten attacks, duration under a minute, stereotyped symptoms and response to carbamazepine or oxcarbazepine. Because vascular contact with the nerve is common on MRI in healthy people, it is not diagnostic on its own.12
Vestibular schwannoma
A benign tumour arising from Schwann cells of the vestibular nerve; its most common symptom is asymmetric sensorineural hearing loss. Because it grows slowly, vestibular loss is often compensated and vertigo may not be prominent.13

Open questions
Is acute unilateral vestibulopathy caused by viral reactivation, immune mechanisms or, in some cases, vascular events? Is there a distinguishing biomarker or imaging finding?
How do we explain the gap between corticosteroids improving the caloric response and long-term symptoms and function? Can a subgroup that benefits be defined?
Can hearing be preserved and long-term adaptation achieved with vestibular implants? Which patients with bilateral vestibulopathy are suitable candidates?
Who develops chronic symptoms (e.g. PPPD) after acute vestibulopathy? Is there an early predictor (see thalamus and cortex)?
