Toplum çalışmasında hastaların yalnızca %8’i etkili tedavi almıştı.3
Posterior kanal BPPV (kanalolitiazis)
Çoklu kanal BPPV: birden fazla kanala uyan nistagmus.

Benign paroxysmal positional vertigo (BPPV)
Episodic, triggered vertigoBárány criteria 2015Nystagmus map · criteria checker · 3D simulatorLast reviewed: October 2026
Intense spinning that starts on lying down or turning over in bed and lasts less than a minute. Otoconial fragments detached from the utricle enter a semicircular canal and make it sensitive to gravity rather than to head motion. It is the most common vestibular disorder and often resolves with a single correct manoeuvre; the diagnosis rests entirely on reading the nystagmus correctly.1,4
At a glance
Lifetime prevalence 2.4%, one-year incidence 0.6%; more common in women, mean age at onset 49. In a population study only 8% of patients had received effective treatment.3
The posterior canal accounts for 80–90% of cases, the lateral canal for 5–30% and the anterior canal for 1–2%; up to 20% involve more than one canal.1
The diagnosis is made by canal-specific positional nystagmus: upbeating and torsional for the posterior canal, direction-changing horizontal for the lateral canal.1,2
Repositioning manoeuvres work: the Epley manoeuvre makes complete resolution four times more likely than a sham manoeuvre.5 Post-manoeuvre restrictions, routine imaging and vestibular suppressants are not recommended.2
About half of patients have a recurrence; residual dizziness lasting weeks is common even after a successful manoeuvre.1,18
Diagnostic criteria
In 2015 the Bárány Society defined four established diagnoses; controversial or newly described pictures were grouped under a separate heading. The criteria combine the history with the nystagmus seen on positional testing.1
Posterior canal BPPV (canalithiasis)
Recurrent attacks of positional vertigo or positional dizziness provoked by lying down or turning over in the supine position
Duration of attacks under 1 minute
Positional nystagmus elicited after a latency of one or a few seconds by the Dix-Hallpike or side-lying (Semont) manoeuvre: a combination of torsional nystagmus with the upper pole beating toward the lower ear and vertical nystagmus beating upward (toward the forehead), typically lasting under 1 minute
Not better accounted for by another disorder
Other established diagnoses
Lateral canal BPPV (canalithiasis): A and B as above; on the supine roll test, horizontal nystagmus after a brief or no latency, beating toward the lower ear with the head turned to either side (geotropic, direction-changing) and lasting under 1 minute.
Lateral canal BPPV (cupulolithiasis): recurrent positional attacks; on the supine roll test, horizontal nystagmus beating toward the upper ear (apogeotropic, direction-changing) and lasting longer than 1 minute. Central nervous system disease must be excluded.
Probable BPPV, spontaneously resolved: typical history and attacks under 1 minute, but no nystagmus and no vertigo on any positional test.
Emerging and controversial syndromes
Anterior canal BPPV: predominantly downbeating nystagmus under 1 minute on Dix-Hallpike (either side) or straight head hanging.
Posterior canal cupulolithiasis: upbeating and torsional nystagmus without latency on the “half Dix-Hallpike”, lasting longer than 1 minute.
Multiple canal BPPV: nystagmus matching more than one canal.
Possible BPPV: positional attacks missing one criterion of the diagnoses above.

Simulation: which test, which nystagmus?
Diagnosing BPPV means knowing which nystagmus to expect in which position. Choose the canal, the mechanism and the affected ear, and watch the expected nystagmus on six positional tests from the examiner’s point of view. To see the same pattern with the patient, the canal and the particles, use the links below the cards to open the 3D simulator.
Loading the map…
Clinical relevance
In the lateral canal, the side is given by intensity, not direction. In the geotropic type the nystagmus is stronger with the head turned toward the affected ear; in the apogeotropic type, with the head turned away from it. To compare intensity, the angle and speed of the turn must be the same on both sides.1
Mechanism
Otoconia are calcium carbonate crystals on the utricular otolith membrane. Detached fragments easily fall into the posterior canal, whose lowest point lies just below the utricular opening when the head is supine; this is why the posterior canal is most often affected.4 Most cases are idiopathic; head trauma, vestibular neuritis, Ménière’s disease, inner ear surgery and prolonged bed rest are secondary causes.4
Canalithiasis
Cupulolithiasis
Particles
Free in the canal lumen
Attached to the cupula, which becomes heavier than endolymph
Latency
A few seconds: the particles take time to start moving and drag the endolymph
None or very short: the cupula is deflected by gravity as soon as the position is reached
Duration
Under 1 minute; stops when the particles reach the new lowest point of the canal
Over 1 minute; lasts as long as the position is held
On repetition
Fatigues (the particles disperse)
Fatigues little
Direction in the lateral canal
Geotropic
Apogeotropic
The direction of the nystagmus follows the plane of the stimulated canal and Ewald’s laws. In the posterior canal, flow away from the ampulla (ampullofugal) is excitatory: the eye beats upward and torsionally toward the lower ear in the plane of the canal. On sitting up the particles slide back and the nystagmus recurs in the opposite direction, more weakly. In the lateral canal, flow toward the ampulla (ampullopetal) is excitatory; the intensity therefore differs between the two sides and shows the affected side.1,4 For detailed canal physiology see Semicircular canals.
Positional tests
Tests are done with Frenzel or video goggles that remove fixation, waiting at least 30 seconds in each position. The guideline recommends the Dix-Hallpike first; if the first side is negative the other side is tested. If the history suggests BPPV but the Dix-Hallpike is negative or shows only horizontal nystagmus, a supine roll test should be done.2
Dix-Hallpike: the patient sits with the head turned 45° toward the side tested and is brought quickly supine with the head hanging about 20° below the horizontal. This brings the lower posterior canal into the plane of gravity.2,4
Side-lying (Semont diagnostic manoeuvre): an option when the neck is a problem; the head is turned 45° to the opposite side and the patient is laid onto the side tested.1
Supine roll test: the patient lies supine with the head elevated about 30°; the head is turned quickly 90° to one side, returned to the midline when the nystagmus stops, then turned to the other side. The intensity on the two sides is compared.1,2
Straight head hanging: the head hangs below the horizontal in the midline; the most sensitive position for anterior canal BPPV.1
Clinical relevance
BPPV is often missed in older people: in a general geriatric clinic sample, 9% had unrecognised BPPV, and these patients had fallen more often in the previous three months. A Dix-Hallpike should be considered in every older patient who reports dizziness.17
Differential diagnosis
Positional nystagmus is not specific to BPPV. The most important distinction is central positional nystagmus, seen with lesions of the cerebellum and especially the nodulus and uvula.15,16
Diagnosis
What distinguishes it from BPPV
Central positional nystagmus
Pure downbeating or direction-changing nystagmus, no latency or fatigue, low-velocity nystagmus that persists while the position is held, no response to manoeuvres, associated oculomotor signs. Greater care is needed with predominantly horizontal or downbeating positional nystagmus; MRI is usually indicated with brainstem or cerebellar signs, atypical features or failure of repeated repositioning.1 In apogeotropic central nystagmus the spontaneous nystagmus is of similar intensity sitting and supine; in lateral canal BPPV it increases supine.15,16Cerebellum.
Vestibular migraine
Periods of positional vertigo are shorter but recur more often, onset is earlier, and migraine symptoms such as headache, photophobia or phonophobia accompany them. Positional nystagmus during an attack may not match a canal pattern. Vestibular migraine.1
Orthostatic symptoms
Occur only on getting up, not on lying down or turning in bed.1
Dizziness only, no nystagmus
May be spontaneously resolved BPPV, “subjective BPPV” or another cause. Repositioning without seeing nystagmus does not confirm the diagnosis.1,16,19

What we know
Posterior canal: Epley
Cochrane 2014 · 11 RCTs, 745 patients
Compared with a sham manoeuvre, complete resolution was 4.4 times more likely (21% to 56%) and conversion to a negative Dix-Hallpike 9.6 times more likely. No difference from Semont was shown; a single Epley was more effective than a week of Brandt-Daroff exercises. No serious adverse effects; nausea 17–32%.5,11
Posterior canal: Semont
Double-blind, 342 patients
Resolution in 79% at 1 hour and 87% at 24 hours after a Semont manoeuvre; none after a sham manoeuvre. Success was higher when liberatory nystagmus was seen during the manoeuvre.6,12
Posterior canal: SémontPLUS
Strupp 2021 · 194 patients
Extending the head 60° below the horizontal when laying the patient toward the affected side shortened the time to recovery (median 1 day vs 2 days with Semont); the success of a single manoeuvre did not differ.7
Lateral canal, geotropic: barbecue or Gufoni
Kim 2012 · 170 patients
Resolution on the first day: 69% with barbecue rotation, 61% with Gufoni, 35% with sham; the two active manoeuvres did not differ.8,21,22
Lateral canal, apogeotropic: Gufoni or head shaking
Kim 2012 · 157 patients
Resolution on the first day: 73% with Gufoni, 62% with head shaking, 35% with sham; the active manoeuvres did not differ.9
Anterior canal: Yacovino
Small series
A manoeuvre that does not require lateralisation; the evidence is limited to observational series.10
Self-treatment at home
Radtke 2004
A modified Epley taught for use at home was more successful than self-applied Semont.20
Not recommended. Observation with follow-up is an option; the patient should be reassessed within a month, and in persistent cases another canal, a coexisting vestibular disorder or a central cause should be sought.2
Vitamin D and calcium (preventing recurrence)
Jeong 2020Meta-analysis 2025
In patients with low serum vitamin D, one year of supplementation reduced the annual recurrence rate by 24% (0.83 vs 1.10 attacks per person-year). A meta-analysis reports lower recurrence with supplementation, but most studies are observational and the effect is smaller in randomized trials.13,14
You can follow each manoeuvre step by step, with the path of the particles inside the canal, in the 3D simulator.
Course and recurrence
Spontaneous resolution within days or weeks is common even without treatment, but the attacks markedly affect daily life and work: in a population study patients reported avoiding leaving home, giving up driving and sick leave.1,3 In older people unrecognised BPPV is associated with falls.17
About half of patients have a recurrence; in the Cochrane review, recurrence after treatment was 36%.1,5
Residual dizziness, felt as imbalance or light-headedness after a successful manoeuvre, is reported in 23–70% of patients; older age, female sex, secondary BPPV, a long delay before treatment and anxiety are risk factors. Vestibular rehabilitation is recommended; vestibular suppressants may delay central compensation.18
During treatment particles can pass from one canal to another, especially from the posterior to the lateral canal.1
Clinical relevance
When a patient says “I still feel woozy” after a manoeuvre, repeat the positional tests first: if there is no nystagmus, another manoeuvre is not needed; explanation and vestibular rehabilitation are enough for residual dizziness.2,18
Cognition: One week after successful repositioning, patients with residual dizziness did worse on the Stroop test; residual dizziness was associated with higher anxiety and disability.23 In older patients the strongest predictor of residual dizziness was pre-treatment anxiety; the MMSE score was not related.24
2025 · Frontiers in Neurology · reviewRecognising residual dizzinessA synthesis of 31 studies puts residual dizziness after a successful manoeuvre at 23–70%; the authors propose a checklist (CLEAR) to screen risk factors and plan follow-up.18
2025 · Frontiers in Neurology · meta-analysisVitamin D and BPPVAcross 60 studies, vitamin D is lower in patients with BPPV; supplementation appears to reduce recurrence, but most of the evidence is observational.14
2025 · Braz J Otorhinolaryngol · reviewNot every positional dizziness is crystalsPure downbeating, non-latent, persistent or treatment-resistant nystagmus and accompanying oculomotor signs call for central evaluation.16
2024 · Journal of Neurology · case and method“Subjective BPPV” revisitedIn a patient without nystagmus on standard tests, an augmented half Dix-Hallpike manoeuvre revealed upbeating torsional nystagmus.19

Open questions
Is “subjective BPPV” without nystagmus a low-intensity form of the same disease or a separate entity? New manoeuvres reveal hidden nystagmus in some patients.19
What determines recurrence? The link between vitamin D, bone metabolism and otoconial structure looks smaller in randomized trials.13,14
Can anterior canal BPPV and posterior canal cupulolithiasis be validated as separate diagnoses, or are some of them central or of another canal origin?1
Is residual dizziness otolith dysfunction, anxiety and visual dependence, or a combination?18
