Unilateral kayıpta, 2000 Hz’in altındaki iki komşu frekansın her birinde kemik yolu eşiği karşı kulaktan en az 30 dB kötü olmalıdır.

Odyometriyle belgelenmiş, unilateral ve ilerleyici düşük frekans kaybı Ménière’i gösterir. Migrende kayıp hafif, sıklıkla bilateraldır ve ağır kayba ilerlemez.1,8
Atakta yüksek hızlı horizontal nistagmus Ménière’i, vertikal nistagmus ise vestibüler migreni düşündürür.6,8

Ménière’s disease is a clinical syndrome rather than a single lesion: spontaneous vertigo attacks lasting hours, fluctuating low-frequency hearing loss, tinnitus and aural fullness. Endolymphatic hydrops is almost always present, but it may be a marker rather than the cause of the symptoms. With no biomarker, the diagnosis still rests on the history and the audiogram.1,3,4
At a glance
Prevalence 34–190 per 100,000; onset usually between 30 and 70 years, slightly more often in women. 8–9% of patients have another affected relative.1
Definite disease requires at least two spontaneous vertigo episodes lasting 20 minutes to 12 hours, audiometrically documented low- to mid-frequency sensorineural hearing loss and fluctuating aural symptoms in the same ear.1
Nystagmus during attacks is fast (~43°/s on average) and often changes direction. A slow-phase velocity above 12°/s strongly favours Ménière’s over vestibular migraine.6
Caloric weakness with a normal vHIT (dissociation) is found in 59% of patients and is highly specific for Ménière’s disease.7
Treatment evidence is weak: placebo-controlled trials of betahistine and intratympanic dexamethasone showed no benefit. Ablative treatments are used stepwise when attacks persist.12,13,14
Diagnostic criteria
The criteria were published in 2015 jointly by the Bárány Society and the American (AAO-HNS), European (EAONO), Japanese and Korean societies. They are the product of four years of work and define two categories: definite and probable Ménière’s disease. The key change from the 1995 AAO-HNS criteria is the audiometric definition of hearing loss.1 The document has not been updated since 2015.17
Definite Ménière’s disease
Two or more spontaneous episodes of vertigo, each lasting 20 minutes to 12 hours
Audiometrically documented low- to mid-frequency sensorineural hearing loss in one ear, defining the affected ear, on at least one occasion before, during or after one of the episodes
Fluctuating aural symptoms (hearing, tinnitus or fullness) in the affected ear
Not better accounted for by another vestibular diagnosis
Probable Ménière’s disease
Two or more episodes of vertigo or dizziness, each lasting 20 minutes to 24 hours
Fluctuating aural symptoms (hearing, tinnitus or fullness) in the affected ear
Not better accounted for by another vestibular diagnosis
Summarised from Lopez-Escamez et al. 2015; the document carries no open licence statement, so the wording is paraphrased with the meaning preserved. See the original for the full notes.1
For unilateral loss, bone-conduction thresholds must be at least 30 dB worse than the other ear at each of two contiguous frequencies below 2000 Hz. For bilateral loss, absolute thresholds must be 35 dB HL or worse at those two frequencies. Recovery of the loss at some point across several audiograms supports the diagnosis.1
Criteria checker
Answer the questions from your patient’s history; the criteria met and unmet appear immediately. The last row shows the core criterion of the probable category separately.
In endolymphatic hydrops the scala media distends and Reissner’s membrane bulges into the scala vestibuli. The simulation lets you follow one attack minute by minute or the disease over years. The cochlear section, the nystagmus in the Frenzel view, the audiogram and the symptoms share one timeline. Switch the affected ear and watch how the nystagmus direction and the audiogram symbols change.

Hydrops: cause or marker?
In a temporal bone study, all 28 ears with classic Ménière symptoms had hydrops, but 19 ears had hydrops without symptoms. The authors therefore described hydrops as a histological marker of the disease rather than the direct cause of its symptoms.4 On gadolinium-enhanced 3 T MRI, hydrops is seen in 93% of ears with Ménière symptoms and in 65% of asymptomatic contralateral ears.1,18 Membrane rupture with potassium mixing into perilymph, proposed to explain the attacks, remains a hypothesis. The extent of hydrops correlates only weakly with symptom severity.5
Clinical relevance
A change of nystagmus direction during attacks is expected: in the home video-oculography series of Young and colleagues, direction changed in 59% of patients with more than one recording. All three phases (irritative, paretic and recovery) were captured in only 3%. The direction in a single recording does not reliably show the affected ear; the side of the hearing loss is a better guide.6
Attack: Often begins with aural fullness, rising tinnitus and reduced hearing, followed within minutes by severe rotatory vertigo, nausea and vomiting. Attacks usually last hours, and the patient may then feel tired and unsteady for about a day.1,3
Triggers: Most attacks are spontaneous. Some patients report excess salt or caffeine as triggers. In advanced disease, attacks of seconds may be provoked by loud low-frequency sound (Tullio) or pressure change, explained by the distended membranous labyrinth approaching the stapes footplate.1
Tumarkin otolithic crisis: A sudden fall or lateropulsion lasting seconds, without loss of consciousness. More frequent in the late stage of the disease.1
Delayed Ménière’s disease: Hearing loss can precede the vertigo attacks by months or years. Conversely, vertigo can precede the hearing loss by weeks, although tinnitus or fullness usually accompanies the first attack.1
Bilateral involvement: The chance of the other ear becoming involved rises with disease duration. Synchronous bilateral loss from the start should raise suspicion of autoimmune inner ear disease, and bilateral loss progressing slowly over years should bring vestibular migraine to mind.1
Course: Attack frequency is highest in the first years and declines over time, while hearing and vestibular loss generally settle within the first 5–10 years.11 This natural decline also explains why treatment studies without a control group can mislead.14
Clinical subgroups: Cluster analysis defined five subgroups of unilateral Ménière’s disease: classic, delayed, familial, with migraine and with autoimmune disease. The subgroups may differ in mechanism and treatment response.10
Comorbidities: Migraine (OR 2.0), arthritis, psoriasis, gastro-oesophageal reflux and irritable bowel syndrome are more common.1
Cognition: Impaired attention, memory, visuospatial and executive function have been reported in Ménière’s disease.19 Cognitive impairment improved at 3, 6 and 12 months after effective treatment, and the improvement was related to vertigo severity.20
Horizontal nystagmus (94%); mean slow-phase velocity 43°/s. A 12.1°/s cut-off separated Ménière’s from vestibular migraine with 91% sensitivity and 92% specificity.6
The most valuable discriminating finding if an attack can be captured
Caloric test with vHIT
Caloric weakness with normal vHIT: 59% in Ménière’s, 17% in vestibular migraine and other disorders. Specificity 84%; without dissociation, Ménière’s becomes less likely.7
Shows cochlear and vestibular hydrops. Because hydrops also appears in asymptomatic ears and in some vestibular migraine, it does not make the diagnosis on its own.1,8,18
Not in the criteria; research and selected patients
Contrast MRI of the internal auditory canals
Excludes vestibular schwannoma, endolymphatic sac tumour and other angle masses.1
Episode duration is the first discriminating question. Ménière’s 20-minute to 12-hour window overlaps heavily with the windows of vestibular migraine and recurrent vertigo of childhood.
Ménière’s disease or vestibular migraine?
This is the most frequently confused pair, and the two can coexist in one patient. The detailed comparison table is on the vestibular migraine page. Three findings strengthen the distinction most:6,7,8
Audiometrically documented, unilateral, progressive low-frequency loss points to Ménière’s. In migraine the loss is mild, often bilateral, and does not progress to severe loss.1,8
When both sets of criteria are met, the Ménière criteria take precedence. Both diagnoses are made only when the patient has separate attack types that fit each.9
Other diagnoses
Diagnosis
What separates it from Ménière’s
Transient ischaemic attack (AICA territory)
The internal auditory artery usually arises from the AICA. Transient auditory and vestibular symptoms precede AICA infarction in up to 42% of cases. Consider it first with recent onset (especially under 30 days), few or escalating episodes and vascular risk.1
Vestibular schwannoma, endolymphatic sac tumour
Excluded by MRI in asymmetric or progressive hearing loss.1,2
Intralabyrinthine schwannoma
Hearing that worsens steadily rather than fluctuating with attacks; needs thin-section temporal bone MRI. On the schwannoma page →
Third window syndromes
Short attacks provoked by sound or pressure, supranormal bone-conduction thresholds and an apparent conductive loss; third window page.1
Vestibular paroxysmia
Many short attacks (usually under 1 minute); response to carbamazepine or oxcarbazepine.16
Autoimmune inner ear disease, Cogan syndrome
Bilateral loss progressing rapidly over days to months; systemic autoimmune disease may coexist.1
Hereditary hearing loss (DFNA9, COCH)
Progressive hearing loss with vestibular loss; family history matters.1
Delayed endolymphatic hydrops
Ménière-like attacks starting in a patient with long-standing profound hearing loss in one ear form a separate entity: delayed endolymphatic hydrops. Years after the original hearing loss, hydrops is thought to develop from late atrophy or obstruction of the endolymph resorptive system.24
Ipsilateral type
Contralateral type
Which ear?
The ear that is already deaf
The previously good, opposite ear
Symptoms
Recurrent rotatory vertigo; hearing does not fluctuate with attacks because it is already lost
Fluctuating hearing loss, tinnitus, fullness; vertigo in about half
Frequency
More common (about 87–89% of cases)
Less common; starts at an older age
Surgical option
Labyrinthectomy or neurectomy is curative (the ear is already deaf)
Destructive surgery is avoided; the only hearing ear must be protected
The preceding loss: most often unilateral profound loss of unknown cause starting in childhood (more than half of cases), followed by viral or bacterial labyrinthitis such as mumps, head trauma, acoustic trauma and sudden hearing loss.24
The interval: from 1 to 74 years; mostly within 10 years after sudden hearing loss and acoustic trauma, and within 20 years after mumps.24
Diagnosis: a history of long-standing unilateral profound loss, subsequent Ménière-like attacks and exclusion of central causes; electrocochleography, the glycerol test, VEMP and hydrops MRI support it. The caloric test alone cannot identify the responsible ear.24
Treatment: first, at least 3–6 months of conservative treatment with diuretics or steroids; vertigo resolves within 5 years in about 65%. For refractory ipsilateral disease, intratympanic gentamicin, labyrinthectomy or neurectomy; for the contralateral type, endolymphatic sac surgery is considered.24
In the clinic
In a patient who says “this ear has been deaf since childhood” and now has new vertigo attacks, the hearing criterion for Ménière’s cannot be assessed; the history carries the diagnosis. In the contralateral type, the only good ear is at risk, and treatment decisions must reflect that.24

What we know
Attack treatment
AAO-HNSF 2020
Vestibular suppressants and antiemetics are used only during attacks and briefly; prolonged use delays central compensation.2
Betahistine
BEMED · 221 patientsCochrane 2023
In the nine-month BEMED trial, betahistine 48 mg and 144 mg daily did not reduce attack frequency in months 7–9 compared with placebo (rate ratios 1.04 and 1.01). Attacks fell in all three groups. The Cochrane review (7 trials, 548 patients) rated the evidence low to very low certainty.14,12
Diuretics, systemic steroids
Cochrane 2023 · very low certainty
Two trials of diuretics were found; the effect on vertigo is uncertain and no harms were reported. Systemic steroids rest on a single small trial (16 patients).12
Intratympanic dexamethasone
Cochrane 2023 · low certainty
10 trials, 952 patients. No difference in vertigo improvement at 6–12 months (risk ratio 1.00). Vertigo days may fall slightly at 3–6 months, but two large unpublished trials reportedly showed no advantage over placebo.13
Intratympanic methylprednisolone or gentamicin
Patel 2016 · 60 patients
In a double-blind comparison, vertigo attacks fell by 90% with methylprednisolone and 87% with gentamicin at 18–24 months. With no placebo arm the absolute effect cannot be separated from the natural course, but equivalence with hearing-threatening gentamicin favours steroid as the first step.15
The effect of endolymphatic sac surgery is debated. Labyrinthectomy is reserved for patients with non-usable hearing in whom other treatments have failed.2
Vestibular and hearing rehabilitation
AAO-HNSF 2020
Rehabilitation for chronic interictal imbalance and hearing aids for hearing loss are recommended.2
Practical approach
Expert opinion
Explain the fluctuating course and the tendency to spontaneous decline; keep an attack diary and serial audiograms. If attacks persist, step up: non-ablative options first (lifestyle, intratympanic steroid), then ablative treatments that put hearing at risk. Treat coexisting migraine and anxiety separately.
The guideline summary is drawn from the key action statements of AAO-HNSF 2020; see the original for the strength of each recommendation and details.2
2024 · Laryngoscope Investig Otolaryngol · 5-year cohortHydrops in healthy volunteers7% of 115 healthy volunteers had asymptomatic hydrops on MRI; it disappeared in half within 5 years, and 2 of 8 developed Ménière’s. Those who developed symptoms had higher depression scores.21
2017 · Otology & Neurotology · smartphone diaryWeather and attacksIn 397 patients, low air pressure (OR 1.23) and humidity above 90% (OR 1.26) were associated with attacks.22
2025 · Frontiers in Neurology · scoping reviewThe need for a core outcome set76 randomised trials used 38 different outcomes and 208 measurement instruments; the COSMED study is developing a common set.23
2024 · Frontiers in Neurology · cohort of 2101The caloric–vHIT dissociation confirmedThe dissociation was present in 59% of Ménière’s and 17% of vestibular migraine. Specificity was 84%; its absence markedly lowers the probability of Ménière’s, and it discriminates better than the caloric test alone.7
2022 · Journal of Neurology · home video-oculographyWhen attacks are recorded at homeAttack videos recorded by patients showed that a 12°/s nystagmus velocity cut-off separates Ménière’s from vestibular migraine, and that the direction of nystagmus often changes.6
2023 · Cochrane · two systematic reviewsUncertain evidence for systemic and intratympanic drugsThe evidence for betahistine, diuretics and intratympanic dexamethasone is of low or very low certainty. The reviews call for a core outcome set across trials.12,13
2016 · BMJ and Lancet · randomized trialsBetahistine no better than placebo; steroid equivalent to gentamicinBEMED showed that betahistine did not reduce attacks even at high dose; Patel and colleagues reported intratympanic methylprednisolone to be as effective as gentamicin.14,15
2016 · Journal of Neurology · reviewHydrops re-evaluatedThe ability to image hydrops with MRI led to a proposal to reframe the classification as an imaging-based “hydropic ear disease”.5
Open questions
Is hydrops the cause of the symptoms or a by-product of a shared process? Why do ears with asymptomatic hydrops never have attacks?4,5
Should gadolinium MRI enter the diagnostic criteria? If so, how should hydrops in asymptomatic ears be interpreted?1,18
How should treatment effects be measured in a disease that declines on its own? Should a core outcome set and long placebo arms be mandatory?12,14
