Solukluk kusmadan önce neredeyse her zaman vardır.12
Birleşme noktaları: Kedilerde mideye verilen bakır sülfat, kaudal vestibüler nükleus nöronlarının yaklaşık üçte birinde istirahat ateşlemesini %30’dan fazla, yaklaşık üçte ikisinde vestibüler kazancı %30’dan fazla değiştirdi. Mide–bağırsak ve vestibüler sinyaller birden fazla bölgede birleşiyor; hareket hastalığı büyük olasılıkla bu bölgelerden gelen girdilerin birleşmesini gerektiriyor.4
İnsanda: 0,2 Hz’in altındaki sinüzoidal galvanik uyarıda deneklerin yaklaşık yarısı bulantı bildirdi.
In a vertigo attack, what wears patients down most is often not the spinning but the nausea, vomiting, sweating, pallor and palpitations that come with it. This is no coincidence: the vestibular nuclei connect directly to autonomic centres in the brainstem. The connection also works every day, helping to maintain blood pressure on standing and to adjust breathing when posture changes.
At a glance
The vestibular system influences blood pressure, breathing and nausea. These effects serve to maintain homeostasis during movement and changes in posture.4,6
The vestibulosympathetic reflex arises mainly from the otolith organs. Canal stimulation, such as caloric testing or rotation in the horizontal plane, does not change sympathetic activity to muscle.8
The vestibular response can begin before blood is redistributed to the legs, which sets it apart from the baroreflex: its latency is 0.4 seconds, the baroreflex’s 1.4 seconds.1,7
The reflex weakens with age, and a weaker reflex is associated with lower arterial pressure.10 In astronauts after 4–6 months on the space station it was absent on return and recovered within 2 months.11
In dizzy patients with absent VEMP responses, orthostatic hypotension was more common: 19.6% versus 8.6%.14
Anatomical pathways
Vestibular and autonomic information converge extensively in both vestibular and autonomic regions. Distinct regions of the medial cerebellar cortex inhibit these circuits, as they do the vestibulo-ocular and vestibulospinal pathways.2
Figure 1 · Vestibulo-autonomic pathways (schematic)Most pathways were shown in cats and rats. In humans canal stimulation does not change sympathetic activity to muscle; the reflex arises mainly from the otolith organs.1,2,3,4,5,6,8
Target
Connection shown
Likely role
RVLM and spinal sympathetic outflow
In animal studies, cells in the caudal vestibular nucleus complex, interneurons in the lateral medullary reticular formation and bulbospinal neurons in the RVLM.1
Vascular tone and blood pressure during changes in posture
NTS and ventrolateral medulla
In rats, medial and inferior vestibular nucleus neurons project mostly to the NTS and fewer to the ventrolateral medulla. In acute hypotension, 38% of MVN neurons projecting to the NTS were activated.3
Visceral reflexes; indirect route for sympathetic responses
Emetic regions
Caudal vestibular nucleus neurons receive visceral input and project to the NTS and the lateral tegmental field.4
Nausea and vomiting, motion sickness
Parabrachial nucleus network
Reciprocally connected with the central amygdala, infralimbic and insular cortex and hypothalamus. The network links autonomic control, vestibulo-autonomic interaction and anxiety.5
Anxiety, avoidance, affect
Respiration
Vestibular input alters the activity of respiratory muscles.6
Offsetting mechanical constraints when body position changes
One view holds that convergence of vestibular and visceral signals (blood pooling, position information from the viscera) contributes to the brain’s internal representation of gravity and acceleration. This representation may affect posture, cardiovascular control, perception of the vertical and emotional responses. This is a hypothesis.2
In cats, nose-up vestibular stimulation increases excitatory drive on the sympathetic nervous system. This offsets the orthostatic hypotension that, for example, vertical climbing would otherwise cause. Cutting the vestibular nerves, in anaesthetised and awake cats, impairs the ability to correct the fall in blood pressure during nose-up tilt.6
Figure 2 · Latency of the sympathetic response after a change in postureDuring off-vertical axis rotation, muscle sympathetic activity followed blood pressure with a 1.4-second delay in nose-down positions, but followed gravitational acceleration with a 0.4-second delay near nose-up. That delay is too short for the baroreflex.7
MSNA decreases.8 The utricle and saccule contribute about equally: MSNA modulation 32% and 29%. Modulation was present even at 1.25 mG, below the threshold for perceiving motion (about 6.5 mG).9
Galvanic vestibular stimulation
Activates the vestibular system non-selectively; given as a pulse train it increases MSNA.8 With sinusoidal stimulation MSNA locks to the stimulus rhythm as well as to the cardiac rhythm.9
According to the review by Carter and Ray, otolith stimulation does not change skin sympathetic activity.8 Studies with sinusoidal galvanic stimulation and sub-threshold linear acceleration, however, showed marked modulation.9
Bilateral vestibular loss: In healthy people, linear acceleration of about 200 mG raises blood pressure and heart rate, and a brisk 260 mG acceleration produces a sustained rise in heart rate. These responses were absent in patients with bilateral vestibular loss.9
Ageing: Ageing, which raises the risk of orthostatic hypotension, attenuates the reflex; the attenuated reflex is associated with lower arterial pressure.10
Spaceflight: In 6 astronauts who spent on average 156 days on the space station, blood pressure rose transiently by 11.9 ± 1.6 mmHg in the first 20 seconds of head-up tilt before flight. This response was absent 1–4 days after return and returned to pre-flight levels within 2 months.11
How much does it matter? The reflex’s real share in maintaining blood pressure on standing in humans is not established. Human studies are small, and translating animal findings to bipedal humans is limited.9,10
Nausea and motion sickness
Symptoms: In motion sickness, nausea and gastrointestinal symptoms come with thermoregulatory symptoms (cold sweating, pallor) and changes in arousal (drowsiness, fatigue). Pallor almost always precedes vomiting.12
Points of convergence: In cats, intragastric copper sulphate changed the resting firing of about one-third of caudal vestibular nucleus neurons by more than 30%, and the vestibular gain of about two-thirds by more than 30%. Gastrointestinal and vestibular signals converge at several sites, and motion sickness probably requires converging input from these regions.4
In humans: With sinusoidal galvanic stimulation below 0.2 Hz, about half of subjects reported nausea. Those with nausea showed greater modulation of skin sympathetic activity, but not of MSNA, consistent with the pallor and sweating that accompany nausea.9
Acute vestibular lesions: can cause nausea, vomiting, tachycardia and palpitations.9
Patients with peripheral or central vestibular disorders show symptoms and signs of autonomic dysfunction. In a subset, vestibular dysfunction has been linked to panic disorder and agoraphobia, and vestibulo-autonomic connections have been proposed as the basis of this link.13
In 7,083 patients at a German dizziness centre, vertigo-related anxiety was lowest in bilateral and unilateral vestibulopathy and highest in functional dizziness, Ménière’s disease and vestibular migraine. General anxiety scales did not differ between diagnoses. The authors conclude that a functioning peripheral vestibular system is needed for vertigo-related anxiety to develop.5
In 248 dizzy patients aged 65 or younger, orthostatic hypotension occurred in 19.6% of those with absent VEMP, 8.6% with normal VEMP and 7.2% with asymmetric VEMP. Men with absent VEMP had a fall in diastolic pressure 1 minute after standing.14
Orthostatic hypotension and the utricle
In 155 patients with orthostatic dizziness who had tilt testing and VEMP, orthostatic hypotension (38 patients) was associated with unilateral oVEMP abnormality, high baseline systolic pressure and heart failure. The longer the oVEMP n1 latency, the larger the systolic fall.15
Orthostatic dizziness
Of 30 patients aged 18–50 with orthostatic dizziness, 56.7% had absent cVEMP and 73.3% absent oVEMP; only 33.3% met criteria for orthostatic hypotension. This is a small study.16
Acute vestibular neuritis
Sympathetic reactivity to orthostatic challenge was reduced 48 hours after onset and recovered within 2 weeks.9 POTS and orthostatic hypotension can occur in the acute phase.17 In healthy people an air-conducted sound changed heart rate during passive tilt; this change was not seen in 7 patients with neuritis.18
BPPV
Patients with residual dizziness after successful repositioning more often had orthostatic hypotension and a larger systolic fall on tilt testing.17
Peripheral vestibular disorders and heart rate variability
In 144 patients, those with both canal and otolith involvement had more functional impairment and autonomic dysfunction. The DHI score was an independent risk factor for reduced heart rate variability (SDNN).19
Bilateral vestibulopathy
No blood pressure or heart rate response to linear acceleration.9
In dizziness on standing, measure blood pressure and heart rate lying and standing; the criteria for haemodynamic orthostatic dizziness rest on this measurement.17
The association between otolith dysfunction and orthostatic hypotension comes from observational studies.14,15 No diagnostic or treatment approach based on it has yet been defined; clinical studies are few.9
Noisy galvanic vestibular stimulation did not change mean arterial pressure or heart rate variability in a double-blind, sham-controlled crossover study of 17 healthy older adults. There is no evidence for it as a treatment for orthostatic hypotension.20
Most anatomical pathways come from cat and rat studies; the region-specific, patterned sympathetic responses seen in animals have not been shown in humans.1
Human studies are generally small; whether the vestibular effect is large enough to prevent orthostatic hypotension is unknown.9
Vestibular-induced orthostatic intolerance and the role of vestibulo-respiratory effects in sleep apnoea are regarded as speculative.13
